Dr. David Hammes is a native of the Fox Valley and was born and raised in Fond du Lac, WI. He graduated from the Illinois College of Optometry in 2004 with a Doctorate of Optometry and returned to practice in his hometown with his father, Dr. Brian Hammes.
Dr. David received the Young Optometrist of the Year award in 2013 for the state of Wisconsin and the Young Professional of Fond du Lac Future Five Award. He has previously served as a Board of Director on the Wisconsin Optometric Association.
Dr. David Hammes participates in the VisionUSA program providing needed eye care to underprivileged children, and the InfantSEE program allows children ages 6 to 12 months of age to receive free vision and eye health screening. Dr. David also participates in Special Olympics and screens athletes yearly at their events.
Dr. David Hammes is particularly interested in ocular disease management and specialty contact lens fits. He counsels patients on wellness and nutritional care to prevent and help treat eye disease and improve general health.
In Dr. David's spare time, he enjoys spending time with his wife, Amy, and their children, Kate and Ryan. He spends much time outdoors golfing, biking, running, and downhill skiing.
Dr. Grant Lepley grew up near Canton, Ohio earning his Bachelor of Science degree from the University of Mount Union in Ohio. He then received his Doctor of Optometry degree from the Illinois College of Optometry in Chicago, graduating in 2013. Dr. Grant returned to Canton to practice for over two years before joining the Family Focus Eye Care team.
Dr. Grant Lepley provides comprehensive eye care to all ages. He is a provider for InfantSEE, a nationwide program in which our youngest patients receive their first eye exam free of charge to ensure they have healthy eyes and are poised to develop normally. He also continues to volunteer his expertise at Special Olympics vision screenings and providing free eye exams to underprivileged children at mobile clinics and is an active member of the Wisconsin Optometric Association.
Outside of his professional life, Grant enjoys spending time with his wife, Julie, and their children, Ella, Mya, and Owen. Julie grew up in the town of Eden, so they are happy to be near her family. He also stays active, golfing, skiing, and playing volleyball whenever possible.
Dr. Cortney Birschbach is a native of Fond du Lac, born and raised. She graduated from UW-Oshkosh with a Bachelor of Science degree with minors in Psychology and Spanish. She then continued to get her Doctor of Optometry degree from the Illinois College of Optometry.
Dr. Cortney's clinical experiences include managing ocular disease, fitting contact lenses, pre and post-operative care following corneal, cataract, and glaucoma surgeries, and interacting with traumatic brain injury patients and those with binocular vision disorders through vision therapy. She is also interested in managing dry eye disease and educating patients on nutritional care and treatment options.
Dr. Cortney is an active member of the Wisconsin Optometric Association. She enjoys playing volleyball and challenging herself with word search puzzles in her free time. She loves outdoor activities and traveling with her husband, Jarred. Dr. Cortney and Jarred welcomed their first child, Bennett, in 2020.
Suzanne Falkowski, O.D., received her Doctor of Optometry from the Southern College of Optometry in 2022 and earned her Bachelor of Science in Psychology and Biology from the University of Wisconsin - Madison in 2017. She has a special interest in pediatric optometry and dry eye.
Dr. Falkowski became an optometrist to make a meaningful difference in people’s lives by helping them see the beauty of the world around them. She enjoys getting to know her patients and finding ways to enhance their vision for the activities they love.
In addition to her work in optometry, Dr. Falkowski is passionate about giving back to the community. As the President of Student Volunteer Optometric Services to Humanity (SVOSH) at Southern College of Optometry, she traveled to Colombia and Belize to provide eye care to underserved communities. She is also a member of the Lions Club and serves as an InfantSEE provider, continuing her commitment to helping others both locally and abroad.
